As the Health Care Finance and Administration (HCFA/TennCare) begins to phase in the Patient Centered Medical Home (PCMH) and Tennessee Health Link (HL) initiatives to further implement the Healthcare Innovation Initiative, they have been talking with hospitals about working to establish a system to provide real time information to the providers who will be designated by the HCFA to provide the intensive care management requirements. Central to this effort is access to ADT information from hospitals to ensure the responsible provider is immediately aware of his patients’ encounters with the hospital, especially visits to the ED. The HCFA has been working with the MCOs to recruit hospitals for providing ADT feeds to the state. This webinar is intended to help hospitals understand the HCFA’s goals and the benefits of connecting their ADT feeds to the state.
